Summary:
As part of the virtual Care Support team, Care Support Specialists play a critical role in helping to coordinate care for palliative care patients, including community resources, hospice, home health and durable medical equipment.
The Care Support Specialists are also responsible for responding to patient and family inquiries, triaging requests to the appropriate local team members for clinical care, following-up as part of the virtual interdisciplinary care team (IDT) on pressing patient care needs, facilitating the communication of patient medical records and visit notes with collaborating healthcare providers and partners, and responding to provider inquiries.
Additional responsibilities may include other administrative functions, as required.
Responsibilities:
Provide medical assistant responsibilities to ensure efficient operation of the field-based IDT.
Assist patient, family, and providers with inquiries regarding enrollment, care, and support, and triage to appropriate team members as needed.
Respond to patient and family member inquiries regarding health plan benefits, including interfacing with local health plan representatives.
Manage referrals from patients, family members, and providers for patient enrollments.
Aid in facilitating the communication of patient medical records and visit notes with collaborating healthcare providers and partners.
Support rescheduling of patient visits, including coordination of rescheduled follow-up appointments that align with clinician and patient availability.
Provide pre-visit reminders to patients and/or caregivers.
Coordinate care for palliative care patients, including community resources, hospice, home health, pharmacy, and durable medical equipment.
Document in the clinical management system and communicate effectively with team members to ensure follow-ups are understood and completed.
Run reports to support interdisciplinary care team performance and operations.
Act as a steward for the organization, professionally representing the business and effectively describing our services.
Participate actively in weekly IDT meetings.
Work with IDT members to support the patient discharge process.
Meet individual and team qualitative and quantitative targets.
Other administrative duties assigned.
Qualifications:
College Degree or relevant work experience.
Knowledge of medical office management systems, procedures, and medical terminology.
Excellent time management skills and ability to multi-task and prioritize work.
Social perceptiveness and service orientation.
Excellent written and verbal communication skills.
Experience with patient clinical documentation systems.
Basic (or higher) level of computer skills, with knowledge of Microsoft Office, including Word, Excel and Outlook.
Medical or pharmacy billing experience.
Preferred
Qualifications:
1-2 years experience as a medical office administrator or providing medical assistance to licensed clinicians.
Experience in palliative care or hospice.
Bilingual or multilingual fluency, including the ability to read, write and speak.
Experience working as part of a virtual team.
