Registered Nurse (Caregiver Support Program)

The Caregiver Support Program Nurse Clinical Assessor reports to the Facility CSP Program Manager or designee. The Caregiver Support Program is part of Social Work Service and work closely with Caregiver Support Coordinators, the interdisciplinary healthcare team and at times with community partners in order to implement and evaluate care. The nurse will be working in the facility, community and conducting in-home assessments Duties include, but is not limited to:
Evaluates and analyzes care provided to CSP Veterans and their Caregivers to assure it is safe, timely, appropriate of high quality and cost effective. Assess, evaluate, plan, implement, and coordinate care, and confer with Nursing and Social Work Service Leadership, as well as other care providers to evaluate and initiate changes that help to ensure quality care. Review/educate the Family Caregiver and the Veteran of skills they will need and determine that the Caregiver has the necessary skills and competencies to care for the Veteran. Verification of the skills and capabilities of a non-licensed Caregiver for purposes of determining if they are able to adequately care for the Veteran and meet qualifications for the stipend. Assess the home for safety, and the need for modifications or additional equipment to facilitate successful care at home. Coordinate with the Veteran's PACT or other specialty team (i.e., SCI or Polytrauma) to arrange in-home services. Follow appropriate mandated protocol for ensuring Veteran's safety and reporting suspected abuse and/or neglect. This includes filing a report with Adult Protective Services to initiate an investigation and to ensure a safe environment for the Veteran. Work Schedule:

Graduate of a school of professional nursing approved by the appropriate State-accrediting agency and accredited by one of the following accrediting bodies at the time the program was completed by the applicant:
The Accreditation Commission for Education in Nursing (ACEN) or The Commission on Collegiate Nursing Education (CCNE). In cases of graduates of foreign schools of professional nursing, possession of current, full, active and unrestricted registration will meet the requirement of graduation from an approved school of professional nursing. OR The completion of coursework equivalent to a nursing degree in a MSN Bridge Program that qualifies for professional nursing registration constitutes the completion of an approved course of study of professional nursing. Students should submit the certificate of professional nursing to sit for the NCLEX to the VA along with a copy of the MSN transcript. (Reference VA Handbook 5005, Appendix G6) OR In cases of graduates of foreign schools of professional nursing, possession of a current, full, active and unrestricted registration will meet the requirement for graduation from an approved school of professional nursing. The following criteria must be met in determining the grade assignment of candidates, and if appropriate, the level within a grade:
Nurse I Level I - An Associate Degree (ADN) or Diploma in Nursing, with no additional nursing practice/experience required. Nurse I Level II - An ADN or Diploma in Nursing and approximately 1 year of nursing practice/experience; OR an ADN or Diploma in Nursing and a bachelor's degree in a related field with no additional nursing practice/experience; OR a Bachelor's of Science in Nursing (BSN) with no additional nursing practice/experience. Nurse I Level III - An ADN or Diploma in Nursing and approximately 2-3 years of nursing practice/experience; OR an ADN or Diploma in Nursing and a Bachelor's degree in a related field and approximately 1-2 years of nursing practice/experience; OR a BSN with approximately 1-2 years of nursing practice/experience; OR a Master's degree in nursing (MSN) or related field with a BSN and no additional nursing practice/experience. Nurse II - A BSN with approximately 2-3 years of nursing practice/experience; OR ADN or Diploma in Nursing and a Bachelor's degree in a related field and approximately 2-3 year's of nursing practice/experience; OR a Master's degree in nursing or related field with a BSN and approximately 1-2 year's of nursing practice/experience; OR a Doctoral degree in nursing or meets basic requirements for appointment and has doctoral degree in a related field with no additional nursing practice/experience required. Nurse III - Master's degree in nursing or related field with BSN and approximately 2-3 year's of nursing practice/experience; OR a Doctoral degree and approximately 2-3 year's of nursing practice/experience. To perform in a fully successful manner, the individual must have the physical ability to perform job-related duties which require lifting, standing, bending, transferring, stooping, stretching, walking, pushing, or pulling without assistance from another patient care provider or significant other. A health examination must be successfully completed prior to this assignment and periodically thereafter as required by the Employee Health Clinic policy. The check can be for multiple reasons such as signing bonus, supplies, etc. The victim will be instructed to deposit the check and use the money for any of these reasons and then instructed to send the remaining funds to the fraudster. The check will bounce and the victim is left responsible.
