Advanced Practice Nurse (Homeless Program)
The APN Provides leadership of a clinic, performing community outreach and case management of veterans in the Homeless Program. Position is supervised by the Section Chief of Homeless Programs and works within the parameter of approved protocols and the department policies and procedures. Duties include but are not limited to:
Facilitates psycho-educational groups including Social Skills, Illness Management Recovery, and Wellness. Provides direct and indirect patient care that includes assessment, evaluation, diagnosis, planning, and treatment of health problems. Responsible to ensure adherence with Behavioral Health (Joint Commission and CARF) standards. Attends mental health-related continuing education programs and maintains current, evidence-based practice. Functions independently and assists the Mental Health Section Chief in planning, administering, supervising and evaluating a program of high-quality nursing care for designated patients. Ensures a specialty focused clinical environment for Veterans with chronic, serious mental illnesses to improve their quality of life. Utilizes Psychosocial Recovery Model and provides evidence-based treatments including Motivational Enhancement. Functions as a member of the Homeless interdisciplinary team guided by a Program Manager and assists team to meet quality indicators and to improve service delivery/customer satisfaction. Provides education, research, and consultation and serves as Coach for assigned clients. Work Schedule:

The Accreditation Commission for Education in Nursing (ACEN) or The Commission on Collegiate Nursing Education (CCNE). In cases of graduates of foreign schools of professional nursing, possession of current, full, active and unrestricted registration will meet the requirement of graduation from an approved school of professional nursing. OR The completion of coursework equivalent to a nursing degree in a MSN Bridge Program that qualifies for professional nursing registration constitutes the completion of an approved course of study of professional nursing. Students should submit the certificate of professional nursing to sit for the NCLEX to the VA along with a copy of the MSN transcript. (Reference VA Handbook 5005, Appendix G6) OR In cases of graduates of foreign schools of professional nursing, possession of a current, full, active and unrestricted registration will meet the requirement for graduation from an approved school of professional nursing. Current, full, active, and unrestricted registration as a graduate professional nurse in a State, Territory or Commonwealth (i.e., Puerto Rico) of the United States, or the District of Columbia. Grade Determinations:
The following criteria must be met in determining the grade assignment of candidates, and if appropriate, the level within a grade:
Nurse I Level I - An Associate Degree (ADN) or Diploma in Nursing, with no additional nursing practice/experience required. Nurse I Level II - An ADN or Diploma in Nursing and approximately 1 year of nursing practice/experience; OR an ADN or Diploma in Nursing and a bachelor's degree in a related field with no additional nursing practice/experience; OR a Bachelor's of Science in Nursing (BSN) with no additional nursing practice/experience. Nurse I Level III - An ADN or Diploma in Nursing and approximately 2-3 years of nursing practice/experience; OR an ADN or Diploma in Nursing and a Bachelor's degree in a related field and approximately 1-2 years of nursing practice/experience; OR a BSN with approximately 1-2 years of nursing practice/experience; OR a Master's degree in nursing (MSN) or related field with a BSN and no additional nursing practice/experience. Working Conditions. To perform in a fully successful manner, the individual must have the physical ability to perform job-related duties which require lifting, standing, bending, transferring, stooping, stretching, walking, pushing, or pulling without assistance from another patient care provider or significant other. A health examination must be successfully completed prior to this assignment and periodically thereafter as required by the Employee Health Clinic policy. There will be intermittent exposure to blood and body fluids.
Estimated Salary: $20 to $28 per hour based on qualifications.
